The U.S. Department of Labor (DOL) has issued the new FMLA forms. They are valid until August 31, 2021. The DOL did not make any material changes to the prior forms. Employers, however, should start using the updated forms, which can be downloaded through the links below.  The new forms also can be accessed from the DOL website.
